He was preceded in death by his father, Edward Lee Perkins.
He is survived by his wife, Tracy Perkins; daughter, Stephanie Perkins; son, Johnathan Perkins; mother, LaVerne Downs; two brothers, Tim (Rhonda) Perkins all of Bardstown, and Steve (Renee) Perkins of Loretto; and several nieces and nephews.
Cremation was chosen with no services.
